Smoketown Family Wellness Center

With life expectancies nearly 15 years below other neighborhoods in Louisville, Smoketown Family Wellness Center has a dream that every child will grow up healthy, living up to their potential regardless where they live. 

This has been an exciting year with the addition of two new pediatricians and our new Managing Director! Insurances and Medicaid pay for traditional medical services and we rely on donations to provide wrap around services to improve overall wellbeing.

Here's how your donation supports families at the Smoketown Family Wellness Center:

• 🌱 $10 = 2 Seed Packets: Grows fresh, nutritious food in our community garden.
• 🍎 $25 = Healthy Snacks: Fills the bellies of children who often arrive at the Center hungry without eating.
• 🩺 $50 = CHW Evaluation: Funds vital Community Health Worker evaluations that aren't covered by Medicaid on the same day a child sees a physician.
• 🚐 $75 = A Tank of Gas: Keeps our van running so our Community Health Worker can drive families to the Center and specialist appointments.
• 📚 $100 = Reach Out and Read: Purchases brand-new books for children from birth to age 5, building home libraries to support Kindergarten Readiness.
• 👟 $250 = Emergency Fund Support: Provides a safety net for urgent needs like medication for uninsured children, clothing, shoes, and unexpected financial crises.
• 🧠 $500 = Specialized Care: Supports crucial psychological or autism evaluations for a child without insurance.
